The Community Health Foundation is to identify needs, raise funds, and award grants to advance health care and the health and well-being of people in the Antigo and neighboring communities served by Langlade Memorial Hospital. The Community Health Foundation acknowledges and supports the Vision, Mission and Values of Langlade Memorial Hospital.

Who We Are - The Community Health Foundation is a non-profit organization that was founded in 1979 by Langlade Memorial Hospital. Its mission is to provide for the health care needs of the community through financial and educational support. Langlade County and portions of Forest, Shawano, Marathon, Lincoln, Menominee, Oconto, and Oneida counties have all benefited from the efforts of the Foundation.

For the last 28 years the Foundation board and its volunteers have worked in conjunction with health care providers to support the health needs of the community.

Where We Are Going - There is no doubt that The Community Health Foundation has affected almost everyone within its service area over the last 28 years. Whether you or someone you know has taken advantage of a balance program, utilized Lifeline, been a new parent in one of our birthing suites, or taken advantage of free education classes, you have experienced the work of the Foundation.

The Foundation board is looking at ways to continue this work in the future. The long-range goal of the board is to ensure that the health care needs of our community are filled for years to come. We are committed to our community and devoted to answering the requests for services that are needed close to home. The Foundation recently collaborated with a consulting group to conduct a feasibility study to determine if our community would be receptive to a capital campaign to help bring radiation therapy to Langlade Memorial Hospital. The results of the study came back a resounding yes. Therefore, the framework is being built to roll out a public campaign in spring of 2007.

Bequest - By remembering the Community Health Foundation with a bequest in your will, you can help ensure quality, state-of-the-art healthcare for generations to come. Your bequest will qualify for an estate tax charitable deduction.

Charitable Annuities and Trusts - These planned gifts can provide a wide range of benefits to you, your loved ones and The Community Health Foundation. For example, charitable gift annuities and charitable remainder trust provide income for life for the donor and/or another beneficiary. They also provide income and estate tax deductions.

Century Club - Members of the Community Health Foundation"s Century Club make a pledge to the Foundation and pay toward that pledge on an annual basis. For example, a $500 pledge could be paid in installments of $100 a year for five years.

Tree of Life - The Tree of Life is a distinctive sculpture located in the main lobby of Langlade Memorial Hospital. It serves as a public thank you to show our appreciation to donors for their generous gifts. The Tree of Life has memorials or honorariums engraved upon leaves for various levels of giving.
